Pyramid Head drags an enormous blade through a fogbound town and cannot be killed, only survived. What separates him from every other unstoppable video game creature is that he is not a threat from outside: he is a manifestation of the protagonist's own need to be punished, and understanding that is the point of the entire game. The design has been imitated for two decades and never bettered.
He cannot be beaten, only outrun, and he is always waiting further in.
He is a projection. When the man he follows finally admits what he did, the sentence is complete.
He cannot be killed and he does not chase. He simply arrives, on a schedule you do not control, because you have earned it.
He is not attacking you, he is sentencing you, and realizing that is the point of the entire game.
